Chapters are merely points in the workout you can jump to (and if you start with chapter 4, for example, the workout will play chapters 4 until the end).
Mix and match is a menu system from which you chose a portion of the workout to do (often a chapter, but sometimes a combination of several chapters which makes logical division of the workout). When you choose chapter 4 on the mix-and-match menu, chapter 4 plays, then you are sent back to the mix-and-match menu to choose another option, and so on.
As to the original poster's question re: chaptering:
All Cathe's DVD's are chaptered, but the earlier ones are less chaptered than more recent ones (and the BB workouts seem overly chaptered in places). Some earlier DVD's are chaptered at the warm-up, cadio portion of the workout, cardio/weight portion of the workout, cooldown/stretch (BodyMax is like that).
Somewhere in the DVD description part of this site, there is a list of chapters for each DVD.
