Caused by age or menapause?

Ladies,
Forgive the 'personal' nature of this question.
I know that menapause can cause painful intercourse.
1)Does it also cause orgasms to be less intense?
2)I know you need to tweak up front to find an HRT that works for the individual. Is it common to have to tweak again after one HRT has worked well for over a year?
