I either step up leading with one foot for 1/2 the climb (he tells us how long it is, when we reach each minute, and when 30 seconds is left) then switch (which seems more intense than alternating feet) or alternate. I have been using the incline part of my Transfirmer for this, so can step higher (about 12") or lower on the incline to increase or decrease the intensity, or "jog" up on to the Transfirmer a bit (stepping down). I also sometimes do straddle steps.
When the hill climb is at level 3, I stay lower on the incline for level 3. For level 4, I step higher and/or do a straddle, and add overhead presses wight weighted gloves.
Before I started using the Transfirmer, I used the High Step set at 12" for the climbs, and for level 4, did the overhead presses and straddles.
