I have this exact same problem, particularly with my left elbow. I am afraid it is that irritated ulnar... whatever. Which basically means a crappy elbow joint. LOL What I have had to do is modify my upper body work so that I am not overextending (which is incredibly painful). This is primarily necessary for tricep work and some back work. I've also found that being very careful NOT to grip the weight tightly helps. For some reason the tight grip is very irritating. I did read here that some folks use foam over the dumbbell grip so that the grip is wider and that helps a lot.
