double leg bar work of LBB, up on toes

When you do the double leg bar work of Lower Body Blast, how high up on your toes do you go? Are you all the way up on the toes only, or are the balls of your feet still on the ground? Any thoughts on what one position does vs another? And if you keep both heels on the ground do you feel like you get more glutes?
 
This is the workout that has made me really curious about barre work. Love this premix so much! I push to the top of my toes on this and since I work out barefoot 99 percent of the time, I feel it really targets my calf area which I want.
 
I've been going to a bar class and when we do drills similar to Cathe's, she always tells us to go all the way up on our toes, as high as possible. I mean, the balls of your feet are still going to be on the ground, but I think it's meant to get more calf.
