How can you do reps to failure and not get injured?

So last Monday I was doing bicep curls and I felt an uncomfortable pull in my left bicep, so I stopped. I laid off the bicep since then and it was feeling better. Today I tried to do a few pull-ups and now it hurts again. What I don't understand is how this happened in the first place. I was completely warmed up and although I was squeezing out the rep, I didn't feel like I was pushing it over the top, just pushing hard. This has never happened before.

So, I guess I'm now wondering, how can you push yourself, squeeze out that last rep, go heavier etc. to grow muscle if you have to worry if something is going to tear? I was just watching "The Biggest Loser" and seeing those people being pushed to their limits and thinking, they aren't injured. Granted, I think they are being pushed too far, but you get my point. Anybody have any feedback on this?
