Offset Weight Training

There is an article in Men's Health about Offset training for faster strength gains. They give an example of one hand lifting say for instance 45lbs and the other 15 or 20 lbs lighter, of course I can't lift that amount of weight! I can't figure out if they mean you should do 3 sets of 8 reps with one lighter and one heavy, or, if you should alternate arms, or if you should do one session with the same arms using light and heavy, and then the following week switch arms. I hope I am making sense. If anyone knows, please share. Also, for you guys looking to make strength gains there is a whole section in the September edition of Muscle Media on how to shake your routine up. One of the things I read, don't know which magazine? said to wrap you dumbbells with hand towels as it recruits different fibers!!
